Any time a student will not be taking Methacton School District transportation when transportation is provided, they must have this form filled out. This form must be submitted to the coach or the athletic director prior to the event.
If a parent shows up at an event and wants to take their child home, they must fill out the form and sign it and give it to the coach before the student goes home with the parent. The forms will then be turned into the Athletics/Activities department. The form must be signed by a parent. If a student is injured and decides to go home with a parent or is sent via EMS, they do not need to fill out the form. However, the coach must inform the Athletic Director and the Athletic Trainer ASAP.
