***PIAA Physical Forms***
Print, complete, schedule a physical appointment with your doctor prior to the first practice. Return to the Arcola Main Office prior to the first practice. If you cannot get an appointment with you doctor, go to a local Urgent Care.
***Sports Eligibility Form***
Print, complete, return to the Arcola Main Office prior to the first practice.
***Arcola Pay to Participate Form***
Print, complete, and follow the instructions on the form to submit the documents and payment.
***Parent/Athlete Letter***
Download and print this document after reading everything else. Must be signed by a parent or guardian and the athlete and turned in no later than your first practice.
